This would be an interesting concept!

Sneaking could be a way for a player to "catch their breath" at high altitudes, and reduce/reverse the oxygen loss!

I agree that this is a very cool idea.

Another possible effect that can come from altitude, is that when the player is above that certain y-level, the same shivering and freezing effect that happens when they are in powdered snow, could also happen. To counteract this high altitude freezing the player could wear leather armor to stay warm, but the higher the player goes, the colder it will get. So, the higher the player goes up, the more leather armor will be required to wear to stay warm. Some sorts of heat sources like the campfire and soul campfire, can also help with the cold. There is a lot of other ideas and concepts that can come from this, so I hope more people comment and add to this!

I believe some effects of altitude sickness should start at around y=110-120, where slowness, weakness, and mining fatigue set in, which resolves after at least a week of staying there. And by y=150-160, you start to see the oxygen bar effect, and after getting used to high altitude, the limit will slowly get higher but not more than around y=220, above which is the death zone. In addition, the effects mentioned above will also get worse with low oxygen and ascending rapidly. Finally, at y=384, void-like damage starts, and the player will lose three hearts every half second, even with a Respiration helmet to prevent players from getting to extreme altitude to AFK long-distance trips.

I think having universal effects like this might be a bit strange. To gate an entire altitude behind it might be rough. Lava lakes or Oceans can be removed and the space where they were can be treated like everywhere else.

Maybe at high altitudes, a block called Mist generates. A hardly noticeable air variant that can be removed by artificial light sources. Being in the mist could very slowly draw oxygen and produce the freezing effect. That way, one could potentially clear the mists and treat the upper altitudes exactly like any other location in minecraft.
