Similar to the option to turn on Hardcore mode, the player could optionally turn on "spawn as baby" or "eternal youth" mode when creating a world. Eternal Youth cannot be changed after world creation, while spawn as baby could be disabled or enabled whenever. The default would be "normal" meaning the player would never encounter this. The natural duration for a player to grow up would be the same as any other mob, but this could be changed with commands, and a basic slider could be in the world creation process. (1min to adult on the low end and 20mins til' adult on the high end for example)
Baby players are slightly larger than a block, being the same size as Baby Zombies. Baby players have 8 hearts, move faster than adult players, can't hold their breath as long as adult players, can ride chickens and other mobs baby zombies can ride, piggy back on adult players heads, and mostly serve as a funny challenge. Whenever the player dies with "spawn as baby" enabled, they will respawn as a baby. Eternal Youth locks the player in as a baby, and any players who join will also be stuck as a baby, this can be paired with hardcore mode for a difficult challenge.
I just think it could be fun to have, it's off by default, and I'd love to see Minecraft have more quirky silly options when making worlds. Maybe give goldy dandelion sus stew the ability to revert adult players and mobs into babies, and goldy dandelions could be eaten by players as well, giving hardcore players a chance.
