For a long while now, players have been begging Mojang for a Sculk Dimension. This concept is what I think is the most vanilla-styled one. & the most lore-rich... possibly.
First of all, the Ancient City's Gate should only be able to be opened with a Warden's Soul. A symbol that you've conquered the darkness. & with this soul, you may open the gate--but at a cost. You will lose two heart containers. You can regain them later through some loot found in the dimension itself. The same loot can be used to permanently gain more hearts than you used to have.
Second, the dimension should be similar to the Overworld--but with the surface extremely elevated, with an incredible amount of cave systems below. The surface is covered in what looks like a fallen version of the Overworld. It's like a parallel reality where darkness wins.
Third the cave systems below should be vast. Not thin like the caves of the Overworld, but always large, filled with many valuable & unique ores. Some come pre-enchanted.
Fourth, there should be at least one new boss added to this dimension. Stronger than the Warden & possibly inspired by the "Spicewood" (a rumored MCD sequel, although unconfirmed & thus possibly a big hoax) Warden boss. These bosses should draw from the power of souls. With each conquered player, they grow stronger in hit strength and resistance. There IS a limit to it, though.
