So caves and cliffs have a could trap blocks coming, how about a few more... Trapped Ladders & Slippery Ladders
Trapped Ladders are crafted with a ladder and a tripwire. While the player climbs the trapped ladder, they have a 5% chance of being knocked off the ladder. So as the player climbs more and more trapped ladders, the odds the player will be knocked off increases.
Slippery Ladders are regular ladders placed on ice blocks. Similar to the Trapped Ladder, the player has a 5% chance of being knocked off. However, this risk can be mitigated if the player is wearing leather boots.
Trapped Ladders placed on ice have a 10% chance of knocking the player off the ladder for each trapped ladder block climbed. If the player is wearing leather boots, the chance reverts back to 5%.
To still be effective against 1x1 or 1x2 shaft ladders, perhaps the player is given some sort of temporary status, like 10 seconds in duration. There is a potion of luck, albeit, it's uncraftable in Java. Perhaps if the player has the "unlucky" status effect they are incapable of grasping trapped ladders and slippery ladders.
Maybe this would justify making the potion of luck a craftable item to reduce the chance of falling off the ladder to 0%
