I think a cleverly-designed dungeon could exploit the mechanisms of ender pearls, along with some redstone, resulting in an interesting structure to explore. Here are some key features:

At the end of it, there is a chamber with a boss able to inflict Mining Fatigue in an area that contains the dungeon, as Elder Guardians do. Alternatively, the effect could come from a new special block. Anyway you MUST suffer from it, otherwise you could enter anywhere, making the following ideas pointless.

The entry is a piston door that opens only if two buttons at each side are pressed very quickly one after the other. The distance between them is such that it’s impossible to press them quickly enough by flying or running with a speed potion, and they are a little bit high up, with a one-block platform below. So you must teleport in order to get in.

Inside the building, you still have to teleport between rooms. There are some other piston doors, this time with only one button each but far from the door. There are also 1×1 windows (cut in obsidian walls), it’s possible to go through them with water or trapdoors but still easier to use ender pearls.

It gets harder to go back as you progress, especially if you run out of ender pearls. But you can instantly teleport outside by eating a chorus fruit which, if eaten in this special place, would instantly teleport you outside, in front of the main door. (But a chorus fruit eaten outside shouldn’t teleport you inside.)
