Totem of Undying should give a "shielding" effect that deflects projectiles and reduces knockback
When you take fatal damage with a Totem of Undying in your hand, it should give a "Shielding" effect (and maybe also Resistance I-II). The "shielding" effect deflects all projectiles and reduces knockback from attacks and explosions for 5-10 seconds, similar to if the player was holding a shield in all directions.
This would make the totem more useful for escaping hordes of mobs. The "Shielding" effect could also be interesting for mapmakers as a way to disable ranged projectile damage for players and mobs.
When a player is shielded, an "aura" could appear around them (like that of Withers and Charged Creepers), but would be more subtle and wouldn't obscure the view in first person.
