Heatstroke.

Maybe you could add heatstroke. Here's how it works..

If you stay in a desert or mesa biome for too long, and don't cool yourself down, you would start by getting blurry vision, then you'd start getting debuffs like blindness, slowness and to fix it, you could add a iced drink to drink to cool yourself down.

    When you have a hat on you should not be able to get heatstroke, unless it’s iron (;
