Starting with the ideas to improve the pale garden...
The sounds of nearby mobs (whether hostile or passive) could be distorted in a way that causes some discomfort.
While the character walks through the pale garden, there could be a new hallucination effect. The hallucinations could spawn mobs around (like the zombie, for example) that would make noises and even attack you, but would disappear when you turn your back (which would be cool, combined with the biome's ability to distort sounds).
Some LogEvent from the biome or the creaking could cause confusion to the character, possibly causing items to shuffle randomly in the inventory.
