If you're too low down in the caves (maybe y level -8 or so), you slowly take damage, sort of like a really light drowning effect with pauses of around 15 seconds. This can be stopped with an enchantment, which could maybe be exclusive to the chestplate. Alternatively, it could be a special armor item (kind of like the turtle helmet). If it is a special armor item, the method by which it may be attained could vary. This would make it so that players can't just rush into where all the diamonds are so easily without having done some early game progression. The coughing effect wouldn't be completely debilitating, but would just make it much more dangerous, and you would have to eat more often to heal, on top of dealing with monsters.
