Disc 11 should be exclusive to Ancient City chests

It seems like the Warden and the Deep Dark were loosely based off of Disk 11, so it'd be cool if you could only find it in the Deep Dark cities

    I like to think that the disk is a recording of someone trying to escape the warden when the city was first invaded by the skulk.
