In Minecraft Set Health packets are never sent when you lose saturation (only when you lose a heart).
But if they are sent to the client it always shows the saturation level (which I found odd because if it never gets sent when you lose saturation why is the saturation data level shown, especially since a vanilla client can never view the saturation level without commands).
Minecraft Java Mods can see this information sent by the server to the client and mod developers can develop an indicator/HUD to show how much saturation you have left before your hunger points starts depleting, This information is never 100% it uses predictions combined with the set health packets sent by the server.
When a player lunges with a Spear it consumes alot of saturation in a short amount of time regardless of the lunge level, this plays a huge factor in minecraft PvP or in general because the less saturation you have the less Health Points you will regenerate, it should send a Set Health packet whenever a player uses the Lunge ability.
