When off handing an item and holding another in your main hand, your attack cooldown resets even though there's no visual representation for that: Let's imagine you're holding a dirt block in off-hand and sword in mainhand. If you place the dirt block, and then immediately try to hit something with your sword right after, it'll be a 'weak hit' as if your attack cool down went down even though you swung your left arm and the attack indicator does not go down. Also for some reason there's no animation, in this exactly described scenario, for your sword being swung, meaning your character can only swing one arm at a time when it comes to animation, maybe intentional, feels like not. Maybe make it so your left hand does not influence the attack cool down in any way, and also make it possible for the player to display the arm swing animation in both arms simultaneosly in situations like these.
