There would be things like thirst, temperature effects, high altitude effects, water pressure effects, fatigue (not sleeping in a while), and wet and dry effects. These could be things part of Hard and Hardcore mode, except thirst, which should function just like hunger in that you can only die of thirst in Hard and Hardcore.
