These "Brimstone Caves" would be a new cave biome filled with poisonous sulfuric gas, which would be a unique type of hazard for the player to avoid. Perhaps through the use of an Armor Enchantment or a new potion effect. The Gas could emit from a source block or just generate in these caves, and I imagine the physics being similar to the Ender Dragon's breath. Also due to the poisonous terrain I imagine mobs wouldn't spawn in the caves, which would also help balance out how dangerous they are. But perhaps they could have an exclusive mob (a Moth or something like that) who are resistant to the poisonous gas. I imaging they would be a neutral mob and wouldn't harm the player unless provoked.
Along with the new cave types, a new "Sulfur Ore" would be added, and even a new type of rock "Brimstone". When mined, Sulfur can be used to craft Gunpowder when mixed with charcoal, and it could possibly have other uses like in brewing. I'm not 100% sure on what Brimstone would do and look exactly, but I imagine it to be a yellow color, and also possibly spawn in the Nether due to the strong association between Brimstone and Hell. Maybe it could even have the property of burning forever like Netherrack.
