When you put an apple in your head slot, it appears strapped on top of your head. It won't provide any defense.
If the apple is shot by an arrow, it splits and disappears. The apple won't slow the arrow down so you could line up your shot and shoot through multiple apples.
It can be used for target practice or trick shots because hitting the apple is a lot harder than hitting the player, especially if the player is moving. But mostly it is just for fun.
Its a reference to the tale of William Tell, who was so good with a bow he could shoot an apple off his son's head. Kinda like Robin Hood who was able to split an arrow down the middle, shooting an apple of someone's head represents an impressive feat and tremendous skill.
