    Awesome idea. Forcing the player to survive in the nether adds an even more challenging character to it. Some ideas of my own:

    1. Don't teleport the player to the nether. There is probably no air at the same position in the other dimension anyways. It somehow makes portals obsolete, is confusing to inexperienced players and op in pvp. Also, this can lead to a lot of lag.
    2. Make the player attract mobs. To prevent the player from just standing still, all neutral mobs should become angry at him and maybe even increase their follow range. Ghasts should see the player through a few blocks, preventing him from hiding in a dirt house.
    3. The effect should not run out, unless the player performs a certain action (drinking milk excluded). This could be for example
    • Killing a wither skeleton
    • Killing a ghast with it's own fireball
    • Bartering a rare item with a piglin
