The flesh cave is a new, creepy sub-biome (much like rivers) That spawns naturally in the Crimson and nether wastes biomes. These new caves are lined with bones, fleshy, pulsating blocks, and a semi-transparent block named mucus. This cave contains many old piglin chests and dead explorers. The cave itself is a living creature, and thus attacking it (breaking or trying to break a block) will alert nearby "mouths" mouths are small, hanging stems that attack the player from above. If the player braves all of this, however the reward will be great, will plenty of gold, and even with killing the tunnel, at a "heart" block. The tunnels are small, and only reach across 1-5 chunks. This won't be too scary, judging by the new ambient sounds, and serves as a new challenge that the nether has been missing.
