The nether is like a deep cave system underground. But what's at the top? When you try to dig up, you will meet the bedrock ceiling. So what about a dimension that could bring you to the top of the nether cave systems?
Of course this dimension will be a lot harder and only for endgame players. The bonus of this dimension could be more ores and some that can't be found in the caves.
Look and Feel
The colors of this dimension should be darker and more red. The "sun" is also very large and has the color of lava and the sky is as red as the normal nether. Smoke particles should spawn on the blocks that are directly exposed to the magma sun.
NPCs
The Nether Topside is too dangerous for regular villagers. Instead you can find pigman traders that can sell you more advanced nether weapons, for example with fire aspect.

Regular pigmen are a lot stronger than the ones in the caves. They have three times the health and a diamond sword that by a relatively high chance has fire aspect 1. These guys can also always break doors because they are more intelligent than normal zombies that would require a special door-breaking-gene.
Ores
In the 1.14 Village and Pillage trailer was a secret: A villager threw an item that looks like blue redstone; cobalt. This ore can be found on the top layer of this dimension apart from regular nether quartz. When mined it drops as cobalt powder. With a bit of luck you can even get it as a crystal.
Weapons
This dimension also brings new weapons that are stronger than diamond tier ones. They can only be obtained by trading with pigman smiths because only they know how to forge lava. Trading currency is not the regular overworld emeralds. The pigmen evolved separately from villagers and use cobalt crystals for trading.

The Portal
It is a larger one than the regular nether portal. It is therefore a lot cheaper to build. No limitations have to be made because this dimension is too hard for midgame players. It is made of 18 red nether bricks. They form a 4x5 square.
Note: I could not make the sky darker or the sun have mor contrast, sorry!

Immediately seek shelter (that is not mado of wood)! Every entity that is not immune to fire will get direct and indirect fire damage
