Effectively all it would do would be to separate the world into Northern and Southern 'hemispheres'. It would just effect how the terrain generates with the far north having taiga, snowy tundra, and frozen oceans while lower down there would be more temperate climates like plains, dark forests, possibly the pale garden if you consider it temperate. At what would be considered the equator of the world you would get tropical climates and desert/dry biomes as well. On that topic it would be neat to see tropical islands in warm oceans. The reason why this would be cool is that this option makes terrain more believable as on more than a few occasions I've been in a desert biome and then out near the coastline there was an iceberg. This doesn't necessarily mean that players would always spawn in a tropical biome either as using some kind of random number generator to determine the latitude range at which the equator appears.
