My thoughts on the Pale Garden is that its a fun concept to introduce more horror aspects into the game, but heavily suffers from how its implemented.
Why does it suffer from being a surface biome? Well it mainly suffers because of the way its horror function only occurs during the night. You can avoid the biome during the night, or clear the creaking heart during the day, making the horror aspect very easy to overcome and diminishes what its trying to accomplish.
How does it fit as a cave biome? Well firstly I think it fits perfectly with the other cave biomes perfectly, it could be a lush cave that failed to thrive, it introduces another entity that focuses on a human sense, such as sight instead of sound. A grim, monochrome biome in a dark environment with ambience easily heightens the fright. The Pale Garden as a cave biome makes it easier to cave as well, since you have a better accessibility to wood making it very useful.
Additional Thoughts; Making the Pale Garden a cave biome would make it function a lot better since you'd be able to consistently have Creakings in the biome (Due to the caves always been dark) rather than the biome exclusive mob being available for a limited time during the night.
Another idea for the biome is to add an orange vibrant vegetation that mimics the eyes of the Creaking to make the player a lot more paranoid, and adds another layer to the Creaking trying to be camouflaged to its environment.
