Losing all your items upon death is too much, you could lose equipment/items that took a lot of work and time to get, and you may not have a replacement for them, this could absolutely ruin the world for a lot of people and get them to stop playing.
Also, it could cause the players to not want to play the actual game and explore because they are afraid of losing their equipment in the process. Some players could work for hours, getting a complete set of netherite armor just for them to be afraid of going anywhere dangerous or even use it at all because they don't wanna lose it.
Suggestion: have an item(placeholder name: "slot protector") which upon use allows the player to permanently choose one inventory slot that will make whatever item in it to not be dropped after death, the player chooses between regular inventory slots, hotbar and equipment slots. I think it's a nice balance between how the game is now while providing some relief to such an extreme penalty to the players who put in the effort to get it, also providing some decision making opportunities as the player has to choose which item to put in that safe slot. Such strong item should be a challenge to obtain. At some point it will make possible that the player will have this benefit for every single item slot but it is supposed to be hard to get and each "slot protector" will only work for one item slot for balance so it will take a very long time to do so, and who gets it honestly deserves it.
