There are quite a lot of potions in the game and even more effects, but it doesn't mean there couldn't be more of them. I have a few ideas, but four seems to be more interesting than the others. Here they are:
1. Combustion: The affected entity burst into flames which cannot be put out until the effect is over. This also makes every melee attacks of the inflicted entity as if fire aspect was used. Brewed with a fire charge.
2. Teleportation: The affected entity would be immidiately teleported somewhere random within a 30 block radius. Would behave like Instant damage and instant health, and by that, I mean that it wouldn't be available as a potion that lasts a certain amount of time. Brewed with a Chorus Fruit or Ender Pearl.
3. Confusion: Players affected with this effect would see their controls inverted as well as their view, which would be switched upside down. For the mobs, it would make them move toward the opposite of where their path wanted them to go. Brewed with Poisonous Potatoes or new items.
4. Freezing: The affected entities would move 5% slower than usual. If a player is afflicted, then their mouse (or whatever else) sensibility would be locked to 15% too until the effect is over. On top of that, the same freezing visual effect applied when in powder snow would be applied upon being affected with this effect. Brewed with Blue Ice and maybe Powder Snow as well.
Well, that's it. I think these would make both solo and multiplayer worlds much more dynamic and fun.
