Works the same as a piston but with deadly spikes.
I think it would be cool to have a slowly closing walls of spikes traps... good thing is there are many resources that would make sense to make spikes with, some that are already spiky like cacti and stalactites.
Balance wise I think they should need an unobscured path and slower speed than pistons starting with a delay that makes a distinct sound to allow players to dodge, block, build, and mine, if the player gets hit it should be half health and decent knockback allowing for a sandwich to get the K.O, sandwich K.O should be able to get most mobs with bigger tougher mobs either needing better material to sandwich or just taking some damage and causing the trap/s to break
In terms of falling on them I think they should multiply fall damage based on the material they are made of and cause a wounded effect possibly negated based on the Mob type or player armour material
Poisoning etc the spikes isn't happening, but if you needed splash potion dispensers you could build them.
