Minecraft doesn't have a lot of utility items to place in offhand, mostly being used for shields or torches. So how about adding in items that can add additional utility options, and also utilize some rarely-used items? Hand mirrors would be crafted by 6 iron nuggets in the helmet formation with one on the bottom center and a pane of glass in the middle. Hand bells would be crafted with 4 gold nuggets in an upside-down Y shape with an iron nugget at bottom center. The uses would be as follows:
Mirrors can be held in the offhand, which while doing so, the player gets a small panel view behind them. Very useful for tunneling to be able to see any creepers that might encroach. Their functionality doesn't end there however: when swung at a projectile such as an arrow or ghast's fireball, it will reflect the projectile to the direction the player is facing. And when used against a ghast's fireball, the reflection window will be larger than usual. This consumes durability however, with mirrors having very low durability (about 20) before breaking. Lastly, mirrors can be placed in an item frame and they would expand similar to a map, acting as an actual mirror in-game.
Hand bells would be used as a way to make noise when rung. When rung, all of the player's pets will abandon their 'sitting' position and warp to the player, helping lost pets find their way home. Also when rung, it will attract attention and aggression from nearby mobs, allowing use to save teammates from being swarmed.
