The ancient cities lack of mobs as we currently only have the warden. So as it is supposed to be scary there could be some mobs like ghosts or mobs consisting of sculk (or something entirely different), that spawn in them. They could be hostile (which would make noise whilst fighting them) or just for ambience/scaryness (sounds, harmless "jump scares", effects on player or surroundings) which would fit the ancient city theme a bit more.
