Difficulty scales with “milestones”, and tells you that enemies get harder, uniques are more common e.t.c, but is random. It happens occasionally, with no clear understanding of what actually changed, how much stronger enemies are, unique chance increase e.t.c. This information should be a lot more explicit. The first game handled difficulty really well, and allowed you to keep getting stronger and face stronger challenges. Higher difficulties also were worth it, as they gave more or new loot, unlike in this game. In settings, you can pick easy, normal or hard, which does nothing but make enemies stronger and make the player weaker, and seems like an odd addition that clashes with the already flawed inbuilt difficulty system, unless you do purely want the challenge.
