There could simply be a unique portal for each dimension:

The End Dimension has its own portal.

The Overworld Dimension has its own portal.

The Nether Dimension has its own portal.
This way, portals would be unidirectional. Therefor, once you travelled to The Nether Dimension for example, you would be trapped there until you built an Overworld Portal to get home (same for The End Dimension).
This system would enable players to build the following:

Nether Portals & End Portals in The Overworld Dimension.

Overworld Portals & End Portals in The Nether Dimension.
-
Overworld Portals & Nether Portals in The End Dimension.
This way you wouldn't need to travel through The Overworld Dimension to journey to The End Dimension from The Nether Dimension (and vice versa). Which, to be honest, doesn't make much sense because they are all different dimensions, and you travel to and from them through magical portals, so why should you be required to travel through one dimension in order to get to another?
I also think that each of these three different types of portals should have the same minimum and maximum sizes. And that they should be able to be built either horizontally (like The End Portal), or vertically (like The Nether Portal).
I also think that the minimum size of these portals should be 1 x 1 blocks (3 x 3 including the frame blocks). Because if the player chooses to build them horizontally, you could just walk up to them and step over the edge into them (or swim into them). You also therefor would be able to construct Portals to be 1-block wide x 2 blocks high, so they could be built in small spaces.
Since End Portals have their own unique frame block, I think other portals ought to as well. I think it makes sense for each different portal to have:

#1: a unique (craftable) frame block.

#2: a unique portal (animation) block.
This would be great so that you can decide where you want to construct your portals in your world (like you can with The Nether Portal). That way you could have a dedicated "Portal Room" in your builds. Regardless of which dimension your build is in.
End Gateway Portals should be removed altogether. "Portals" should be for travelling between different dimensions, not within dimensions.
Just eliminate the ~1000-block distance between the Central End Island & the "Outer" Islands. There would simply be a large 'central island', surrounded by smaller islands.
That way, if players want, they can postpone their fight with the End Dragon until they've explored some End Cities and secured an Elytra, making the fight with the End Dragon more fun.
