Since minecraft created a whole new game mechanic called ominous events but there's only two at the moment, it makes sense if they add more. Ancient cities seem like a structure that would get an ominous event.
Maybe when you drink an ominous bottle, you could get warden omen, which makes wardens come out of the ground all across the ancient city. These wardens wouldn't despawn until you broke the sculk chanellers, sculk blocks that are spawned by the ominous event. When all 5 sculk chanellers in the ancient city have been destroyed, the wardens could despawn and some cool stone chests could emerge out of the ground in various locations. These could have normal ancient city loot but also some of the rarer items in ancient cities, like enchanted golden apples and the armour trims. If the center structure turns out to be a portal, maybe you could find the key to unlocking that too ...
