This item would work similarly to how a Totem of Undying works. When the player is in a bit of an iffy situation and has gotten below 3 hearts, the Survivor's Totem would activate and give an adrenalin boost, a bit like the flight-or-fight response in animals when they're in danger. IRL, adrenaline increases the heartbeat and blood flow to muscles which make animals faster/stronger for a short period of time.
The Totem would give you Strength I and Speed I for 5 secs. However, since you are using up more energy, your hunger will deplete twice as fast for these 5 secs (for balancing).
Mob drop and very rarely in chests (Not sure which mob could drop it though)
Rechargeability (Not sure which is better):
Used once and disappears (Like the Totem of Undying)
Used multiple times but needs to be recharged over some time every time it gets activated. For eg. (Used once, recharges for 2 minutes, can be used again) OR (Used once, used twice, used three times, recharges for several minutes, can be used again).
I definitely see this being quite useful when you're in a stressful situation and about to die. You could either attack the hostiles or run away from them in an attempt to survive.
PvP-wise, I think it could make some pretty interesting combat.
