I had an idea, a weird one. Here we go.
The Bloodgrub would be silverfish-sized end or nether creature. It would be a neutral mob which doesn't attack you, not even if you hit it. It attacks then it smells blood. When a player takes damage within a certain radius (15 blocks?) of a Bloodgrub it attacks, hopping along the ground (rather like a slime). If you can avoid taking damage for the next 30 seconds it will forget you and go back to being neutral. The timer resets every time you take damage in their radius. When the Bloodgrub is angered it has a chance of spawning one to three more Bloodgrubs that will be spawned hostile towards you (for the next 30 seconds).
