It would add a dimension that is aimed at very late game worlds, featuring a harsh and dangerous environment adding nothing but a rough plain of cobblestone and pure darkness. Emitted light gets devided by 2 and being in complete darkness for more than 5 seconds deals massive damage regardless of armor. Hunger gets eaten twice as fast as in the overworld. Players dont regenerate health naturally and potions only give half the healing effects. Golden apples cannot be eaten here.
Blue fire however (soul torches and lanterns...) has a lighting Radius of 30 blocks, being the only good source of lifesaving light.
Mobs dont spawn in this environment, however, hostile mobs can "Fall" into this dimension of they die after killing a player. Hostile mobs have 4 times the health in this dimension.
Beds will not work, however there will be a loadstone equivalent fueled by either sculk blocks or something similar.
Personally i would make it only accessible through the nether so it gets the flair of being even more end-game.
Animals like Cows and pigs can not survive there and take constant damage. Plants do not grow.
The dimension adds an effect that deals 0.5 hearts of damage every 10 minutes. That can be turned off by wearing an armor part made of warden stuff.
As a reward (other than the pure coolness factor) ores might spawn more often than on the overworld. Maybe even twice as much, giving a high risk high reward situation.
